Services provided for:

Anyone between the ages of 0-18 (19 if enrolled in school)

Any child, even if we are not their primary care provider
-
Any child – regardless of the ability to pay, insurance status, immigration status, etc.
Access:

Walk-in or make an appointment

Anyone aget 14 and over can consent for their own treatment for mental and behavioral health services
-
Anyone age 15 and over can consent for their own medical care
-
Anyone meeting the above age requirements can request their visit be confidential, which means the patient gets to decide who gets access to their health information
Payment:

We offer a Sliding Scale Fee, which often results in zero cost to patients and families
-
We connect patients to insurance when possible
-
We have access to patient funds, which may assist with transportation or costs associated with referrals for additional services outside of Mosaic
